The Indiana Orthopaedic Center looks forward to having you join our host of satisfied patients. Each of our orthopedic doctors has a schedule tailored for your convenience. Once you have made your appointment you can prepare for your appointment in advance.
Here is what you will need to bring to your appointment:
- Your insurance information including the Insured’s (who has the policy) social security & date of birth through our secure portal. (Remember to bring your insurance card with you to your appointment.) This is preferred. Your chart will be ready prior to your arrival.
- List of current medications.
- Dates of previous surgeries (if any).
New Patient Registration
There are 3 ways to supply your information for registration.
- At the office: You will need to arrive 30 minutes early on the day of your first appointment to fill out forms.)
- Print out registration forms below, fill it out & bring it in at your first appointment.
- Click here and register online through our secure portal. This is preferred. Your chart will be ready prior to your arrival.
